Eligibility criteria for patients enrolled in Medicare Parts B and D

Eligibility Requirements

In order to be eligible for the program, you must meet the following requirements:

  • You must be a resident of the U.S. or U.S. territories and be under the care of a licensed healthcare provider authorized to prescribe, dispense and administer medicine in the U.S. or U.S. territories

  • For patients with Medicare Part B, you must have no access to the prescribed product or treatment via your insurance or supplemental insurance coverage.

    For patients with Medicare Part D, you must have no access to the prescribed product or treatment via your insurance or not be eligible for or enrolled in the Low Income Subsidy (LIS).

  • You must have an annual household income of 400% of the current Federal Poverty Level.
